Complaint Form

This is a case-specific, personal complaint form to be used, for example, when you wish to claim wages you believe your employer owes you within the past year. 

  • please print, fill out, and return to the Labour Standards Branch  


Anonymous or Third Party Complaint Form

This form is to be used when you have some evidence that an employer may be in contravention of labour standards laws and you do not wish your identity to be known. For example, Labour Standards normally will educate the employer regarding such items as the overtime laws or how to calculate proper Public Holiday pay. A short time period would be audited for all employees affected. 

  • download a PDF form to print (8.5x14 legal size), fill out by hand, and send to the Compliance and Review Unit by mail or fax

  • download a Microsoft Word Document form (8.5x11 letter size) to print OR fill in on your computer and later attach to your e-mail to the Compliance and Review Unit


Averaging of Hours Permit Application

This permit facilitates an averaging of the hours of work for employees who to want to work longer shifts but over fewer days or weeks. This does not eliminate overtime.
